The America’s Health Rankings model synthesizes measures across five categories — Social and Economic Factors, Physical Environment, Clinical Care and Behaviors — reflecting that determinants of health directly influence the fifth category, Health Outcomes. This model is built upon the World Health Organization’s definition of health: “Health is a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity.”
